Memory Cart Appointment Request Form

Please note that submission of this form only requests usage of the Memory Cart. All appointments are pending until confirmed via email. 

Briefly describe the project you are hoping to the use the Memory Cart for. For example "digitize print photos" or "scan slides" would be acceptable. 

Please tell us your preferred date and time to use the memory cart. The Memory cart is available during the following time slots:

Monday - Thursday

10am-12pm, 1pm-3pm, 4pm-6pm

Friday & Saturday

10am-12pm, 1pm-3pm

Submissions need to be given at least one week in advance.
